Please plan to arrive 15 minutes early for your first visit to the Embarcadero 4 Dental office in order to complete a registration form and medical history. If you have recent dental x-rays you may wish to bring them with you. If you are being referred to Embarcadero 4 Dental from another dentist, please bring your referral form or letter.
We are here to serve you
During your first visit, we will do a thorough and comprehensive examination, including an oral cancer screening. We will gather all necessary records, which may include a full set of X-rays and models. All information that we obtain will be carefully evaluated so that we can customize a treatment plan in order to meet your individual needs.
Preventive Oral Hygiene Appointment
At your next visit, you will meet your hygienist. We consider this session to be one of the most valuable services we can provide to our patients. At this time, we will personalize your home care to maintain your gums. After all, your healthy gorgeous smile begins with a clean mouth.
Dental Evaluation and Plan for Treatment
After your hygiene session, we will present your treatment plan, taking your immediate needs as well as your long term objectives into consideration. We will always recommend the best treatment options. Where there are choices, we are happy to discuss options with you. Our goal is the help you keep your teeth for a lifetime. Our treatment recommendation will be based on maximizing the life of your teeth.
You are encouraged to ask any questions you have concerning your dental care and its potential costs.
Appointment confirmation
As a courtesy, our office will confirm you appointments by telephone or email. Your appointment is time dedicated to you. If you cannot keep an appointment, please inform us at least 48 hours in advance, so we can accommodate another patient.
Referrals
We’re always happy to meet new patients. And it gives us great satisfaction to know you believe in our services. The greatest compliment we can receive is a referral of a friend or loved one.
Financials
Payment is expected at the time services are performed. We accept cash, checks, Debit, Visa, MasterCard, Discover or American Express. When extensive dental care is necessary or when dental insurance is involved, financial arrangements can be made with our office manager. Please inquire about our financing plans. Our primary concern is your dentistry. Nonetheless, we will be sensitive to your financial circumstances within the framework of sound business practices.
A Word About Dental Insurance Our accounting department will prepare all the necessary forms for this important benefit. However, we remind you that your policy is an agreement between you and your insurance company, not between your insurance company and our office. We can make no guarantee of any estimated coverage, but we’ll do our best to see that you receive your maximum benefits. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.
Cosmetic Dentistry and Insurance Cosmetic procedures are generally not covered by most dental insurance policies; however, partial benefits are often available. For example, most insurance companies will pay approximately 80% of the cost for silver fillings on posterior (molar and bicuspid) teeth. If a more expensive porcelain or tooth-colored filling material is used, the insurance company usually still pays their percentage based on the cost of the less expensive silver filling.
Treatment is always based on firm mutual understanding.
